Registered Nurse
Under the direction of the Agency’s Director of Professional Services (DPS) and/or the Clinical Supervisor, provides skilled nursing services in the home in accordance with the State Nurse Practice Act, accepted professional standards of practice, regulatory guidelines and licensure requirements; and maintains compliance with Agency policies and procedures. Meets, maintains and supports the standards of nursing care and practice in order to meet the healthcare needs of patients, their families and the community.

Home Health Administrator
Accountable to organize and manage all aspects of administrative and clinical activity in one or more agency locations. Responsible for growth of agency business, financial management, and integrity of service delivery to meet regulatory, company, and professional standards. Responsible to empower and challenge staff, recruit, train, and retain the best people and provide resources and environment for people to be successful.

Home Health Coordinator
Establishes and maintains ongoing agency contacts with area health facilities and community organizations, facilitate patient/client transfer between the institution and the home, and assure Company involvement in complete and continuous patient/client care consistent with regulatory compliance and Company policies, procedures, and standards. Initiate pre-admission intake, including completing all Company required forms and obtaining physician’s orders and contributing to development of the plan of care.

Home Health Aide
Responsible for the provision of personal care and designated health-related services in compliance with Live Long WellCare policies, procedures, and the personal care plan, as well as applicable healthcare standards, and governmental laws and regulations. The Home Health Aide functions under the direction of the registered professional to proactively meet the needs of, and assist the patient/client in, the achievement of individualize defined and measurable outcomes.

Licensed Practical Nurse
Under the direction of a Registered Nurse (RN), provides skilled nursing services in the home in accordance with standards, regulatory guidelines and licensure requirements. Maintains compliance with Agency policies and procedures. Meets, maintains and supports the standards of nursing care and practice in order to meet the healthcare needs of patients, their families and the community.

Certified Nursing Attendant
Provision of personal care and designated health-related services in compliance with Agency policies, procedures, and the personal care plan, as well as applicable healthcare standards, and governmental laws and regulations.

Clinical Manager
Accountable for the clinical management functions, employee performance, and patient assessments to assure delivery of quality health care and services consistent with regulatory compliance and Agency policies, procedures, standards and financial objectives. Responsible for the overall coordination and case management oversight of the Agency patients. The incumbent mentors clinical staff for the delivery of quality, efficient, and effective services.

Business Services Manager
Responsible for the management of services to support Agency operations in the areas of payroll and billing processes, customer support, personnel file maintenance, medical records and data entry. This position transmits information to staff and office personnel to facilitate daily office activities and ensure that sensitive company information is appropriately handled.

Director of Professional Services
Responsible for direction of all home health clinical services, including, but not limited to supervising home health staff, oversees client care, ensures the implementation of the plan of care for each client and ensures that the living environment is sanitary and comfortable for the client under the direction of the Administrator. Establishes, implements, and evaluates goals for providing home health services that support the mission and philosophy of the Agency.
